Choose "New Quiz" but it always seems to create Form - Never have option to provide answers

After Choosing "New Quiz" it seems to just create a "From".  Quiz options like being able to provide an answer to the question are not available and there is no "Quiz" option in the ... menus.   I have tried many times and I always get the same result.  Can figure out how to actually create a Quiz.   See below:
