Added new questions to existing MS Form but it is not visible at user end

Try deleting the last question on the form and then re-adding it. I had this same issue and found that tip here and it worked for me: https://techcommunity.microsoft.com/t5/microsoft-forms/questions-missing-when-i-preview-form/m-p/203857
