Do you perhaps have a conditional access policy the requires that when registering security info MFA is enforced?
- Nope, as per my original post, all CA MFA policies are either set to OFF or Report-only

What does your sign-in logs says? Do you have any CA policies enforced?
Do you have security defaults enabled?
