Item level permissions report

For a) I would recommend to use a third party tool such as SharePoint Documentation Toolkit....it's great to get this information. In regards of b) not sure what you mean
jcgonzalezmartin for b), here is an example of a situation - image a site has a few libraries where the owner of the site has given contribute rights to certain individuals. the "Full control" permissions on those items are retained only by this specific site owner.
Now, the owner has moved out of the company and no one else (except Site collection admin) has "Full control" permissions on those items.
So were looking at the best way to identify those items and provide "full control" permissions to the new owner in a wasy way,
