Employee appraisals with Office 365

Hello, has anyone used Microsoft Forms and Flow to set up an effective employee appraisal process? I've done the form and tried to link it within Flow but got lost. Basically what I need is this process: when an employee completes the form, the manager is notified and is able to open that form and complete a section that is for managers only. Once that's done, the form is completed. 

Is it possible to achieve it within Microsoft at all?
