Trying to link cell toanother cell so both things will be shown as together in combined function

I am trying to Create database with names. You can see combined function, so my list will be sorted in alphabetical order according to surnames, but I need in that list also displayed corresponding first name to that surname.

Also, I need to link all the dates in one line to that specific surname, so whenever I add another name, the dates will move with that corresponding surname.

Can anybody help?
