Interactive form

Could Please take a look at the question on this link

Basically, it is a multiple-choice question where the respondent checks only those items that are important/relevant to them and only those items are then 'open' for them to rate. In this way, the respondent only has to respond to those items that are relevant to them.  

I want to create this type of question in an excel sheet ....is it possible, if so how????

    The referenced website requires a log-in using an email address.
    I won't do that.

    I would guess that creating an interactive form in Excel would require the use of VBA code.
