I need help writing a complex formula

Dear people,

I have just started a new postion which i have to expert in Excel. I have some data regarding the trainings dates and i have to fill them in a new excel form in order to upload them to a new system. that means i can't change the form in any how and the old data must fin in the new form? i hope any one can hepl me to find a good solution.
