Excel Table data for "roll up" report

Hi,

I am trying to store location details - specifically issue tracking in a table and would like to be able to have a formula on a separate sheet (same workbook) roll up all noted issues.  I have attached a sample worksheet with an example of what I am trying to accomplish.  I'm very new to Excel and appreciate any help.
