Easy formula to simplify form

Hey everyone !&nbsp;I really need your help! it's been 2 hours that i'm looking to find a formula to make my life easier with this excel form and I don't know where to look anymore&nbsp;I tried with the formula IF but I need it to work with Text and not numbers&nbsp;&nbsp;I have a list of members and a level of identification for everyone.&nbsp;&nbsp;In my new excel form i'm looking for a formula that will automaticly put the level of identification next to the member I will enter.&nbsp;&nbsp;Is it making sense? Is it possible?&nbsp;Thank you for your help!&nbsp;&nbsp;

the formula could be
=IFERROR(INDEX($C$3:$C$8,MATCH($F4,B3:B8,0)),"no such member")
