Creating Dependent Cascading Drop-Down List To Build A Rating Profile

Hi there,

I am looking to create a profile and rating spreadsheet using cascading responses i.e.

Question 1: Do You Perform An Assessment?

Answer is limited to two (2) options: NO or YES

the response of NO or YES cascades in pre-selected options for question 2

Question 2:  How Often Do You Perform An Assessment?

Answer is limited to three (3) options depending on the answer provided above:

NO = NO ASSESSMENT

YES = YES [annually] or YES [once per month]

Question 3: What Rating Is The Assessment Given?

Answer is limited to number range choices depeding on the answer provided above:

NO ASSESSMENT = choice of number ranging from 0 - 3

YES [annually] = choice of number ranging from 4 - 5

YES [once per month] = choice of number ranging from 6 - 7

In a forth excel cell, I need a formula to determine based on the number value established in Question 3, if answers meet one (1) of three (3) profiles:

SILVER = number ranging from 0 - 3

GOLD = number ranging from 4 -5

DIAMOND = number ranging from 6 - 7

Any help would be greatly appreciated!
