List on a page misbehaving

Our site, EApp (for electronic application) takes kiosk created applications for assistance and makes them available online for county personnel to process. The page this list was on was hacked, and I've been unable to recreate it as it is documented in the user manual.

The problems are
1) the list embedded on the page does not show the name of the folder (county) but used to and needs to.
2) the list doesn't show/make available the different view options
3) the list gets stuck on the last view. I.e., refreshing the page does not take you back to the start - it's stuck on the last view the user had.

Thank you for your help - there are many crabby users waiting for this to be fixed.
