Planner task when a Microsoft Forms is submitted- how to resolve error?

Hello, I am trying to create a Planner task when a Microsoft Forms is submitted. However, I keep getting the below error. I know I am doing something wrong in Update Task Details. Can someone please help me out?

@kanwalkheiri if you are creating a task in Planner then updating that task with the description etc you must always put in a delay action of about 10 seconds between the two. The reason is that the flow actions run so quickly that creating the planner action will not have completed before the update item tries to run, so it won't find the item.
