You can add images/pictures in questions, themselves but you can’t add images in multiple choice questions. There are multiple uservoices open for this including this one

Would recommend you vote on it in order to push it up the agenda.

Sebastien75019 that's an old post and since then the uservoice sites have been replaced by the official Microsoft Feedback Portal which for Forms is at https://feedbackportal.microsoft.com/feedback/forum/0ab9de26-ff1b-ec11-b6e7-0022481f8472.
The topic is based on the old Forms Pro which is now called Customer Voice and requires a Dynamics 365 licence.
It is not possible to add an image to each option in a ranking question.
