Microsoft forms crashing when you try and submit

Hi All,

Our team has been experiencing an error with a microsoft form that we developed. The form has around 50 questions and under 5000 responses. Upon completion of filling out the form, when clicking on submit the form will crash and come up with the following error message:

Has anyone experienced this before and could provide assistance?
