ahmedusam you can't pre-populate the audit name or any other information in a Microsoft Forms form; you would need a different form for each department. If you wanted all the responses from all departments collated into a single list you would need to build a flow in Power Automate that saves each response into a SharePoint list with a column that shows which department submitted it.
