Building questionnaire with dropdown

Not sure i am phrasing the subject of my request well but i want to be able to build a survey using this type of control as described here

sessime that type of control is not available in Forms.

May be Powerapps can do this?
sessime No, PowerApps has dropdowns and combo boxes but not the ability for the user to drag items into the order they want.
