Pivot Chart 4 columns, set responses to 4 questions

Hi All,

Feel like I'm going crazy with something that should be simple but which never seems to work for me.

I have data that is set out like so...

I want to create a pivot that looks something like this:

You'll note that the counts are wrong in the above example.

And this is how I'd like the chart to look, basically, but I can't get the counts to be accurate. What am I missing?

Have attached sheet to give example.
