Insert column in a table containing merged rows and un-merged rows

Have a table with cols with merged rows (varying) and un-merged rows.  Need to insert a column with merged rows, exactly like the one existing.  

In the sample file, i need to add a column exactly like Patient Name.  Need help, thank you for your advice.
