building a form in excel.
I started a file in excel and make a table and now want to build a form, I followed the instruction but when I add form to the custom ribbon and say ok. it does not show up on the ribbon or tab's
What am I doing wrong

All is correct. Form is created in Forms, you won't see it in Excel ribbon. Just all answers will be collected in Excel file from which you generated the Form.
