The hotkey for paste unformatted text has stopped working?

I used to be able to copy (ctrl c) from any source such as another Word document or even the internet, then press paste (ctrl v) then hit (ctrl) release the ctrl key then press T and it would turn formatted text into unformatted text.  This hotkey no longer works and I'm trying to figure out how to get it back.
