This is a classic failing SSD with hardware-level read errors - not a software or driver issue. When disk activity shows 100% but progress remains at 0%, the drive is stuck in retry loops trying to read bad sectors.
It turned that the underlying problem was that the laptop was only crashing while being charged. The first time I turned it on while on battery it booted into recover mode where I ran a chkdsk /f /r. It made a few corrections then was able to boot into Windows on next restart. I'm not sure if the 100% activity was caused by the errors, or if that particular SSD doesn't like my particular M.2 enclosure, but I have it working now.
