How does my battery drop from 40% to critical in 30 seconds?

My laptop battery suddenly plunges from 40% to critical in about 30 seconds, which makes the machine basically unusable without being plugged in. Has anyone seen this kind of instant drop before, and could it be a calibration issue, a failing cell, or something in the power settings?
