How can a laptop battery report 100% but shut down the moment you unplug the charger?

My laptop shows a 100% battery charge, but it shuts down immediately when I unplug the charger. I've tried restarting it and checking the battery icon, but the problem continues; what could be causing this, and how can I tell whether the battery needs replacing or there's another issue?
