Unable to add new items in sharepoint list
Do you see +New button at the top of SharePoint lists to add new items using list forms?
Or do you see Edit in grid view button to add multiple list items at a time?
If you have permissions and those buttons are not visible, check if anyone hide those buttons using the SharePoint JSON formatting command bar customization options.

Thanks, but I have those buttons and once I select those buttons it opens a window where I should be able to add new item but I cannot see anything there. I have added the image if it helps.

- Try clearing browser cache & cookies (for All time) and re-login to SharePoint and see if the list form loads properly OR try using another browser/incognito window, etc.
- Check if list form fields/columns are hidden (maybe conditionally). Refer this Microsoft documentation: Show or hide columns in a SharePoint list or library form
