If users must retain Create Items permission, hiding the command bar may not be enough because the new web part title menu can still expose creation entry points. I would test whether the behaviour changes when the page uses a view created specifically for embedding, with only the columns and formatting needed for display, and with your approved intake form linked separately on the page. Also check whether the list web part is using the new experience rollout in your tenant by comparing a targeted release user and a standard release user. If there is no supported toggle, open a Microsoft 365 support ticket with the page URL, list view settings, permissions model and screenshots showing the unwanted New Item path.
