dayra_nz Did you try switching to "New experience" from advanced settings? You have to use new/modern experience to see forms like image 1.

Also, did you add any classic experience column in your list like Location column, etc.? It might be the reason forms changes to classic experience.

dayra_nz Can you share screenshot of "Columns" section on list settings page for all columns and their data types?
