Integrated - PowerApps Customized Form - not working

Hi,
I've removed the datasource and added it back in but still getting the error message, when trying to open the customised form.
I selected the 'learn more' button and a page from learn.microsoft opened with a list of required services.

Does anyone know if any of these are specific to the issue?
