Ideas requested

Hello everyone,

Background:

I am looking for ideas on how (if) I could use SharePoint to address several requests I've received at work to gather information.

I work in a Finance department and each month we have questionnaires that must go out to different individuals within the organization to gather data and other feedback on.  The questionnaires (which come from Corporate) do not always include the same questions, though often times 85% of them are repeat questions.  Questions within the questionnaire apply to various key individuals within the organization, and this varies depending on what is asked.  The questions do not ALL apply to the same individuals, meaning person A may be responsible for answering questions 1 - 3, 5, and 10, while person B may be responsible for answering 3 - 4, 6 - 9, 10 - 12 (there is some overlap).

My request:

Is there a way to establish a list/form in SharePoint where questions on it can be setup and assigned to the key individuals involved (so they only see theirs and no one else's), then allow for them to complete their respective answers and merge all responses together?

I want to provide them with a link that takes them to the SharePoint site where they see ONLY their respective questions, have the opportunity to respond, and then I can automatically gather all responses back into a consolidated format to send back to corporate.  Also, it would be even better if I could assign a due date and SharePoint automatically email/hound them to complete their questions by that date/time.

Hopefully this makes sense, sorry if it's confusing in any way.

Current State:

Currently I am cutting out all of the questions from within the original word document, sending questions to each of the key individuals, hounding them for responses as needed to make the due date, and then stitching all of their answers back together into the final document at the end, with responses color coded by who they belonged to.
