How to make default template for new wiki page

Hi , 

I'd like to use SP wiki page to create new content.

Every time, when a new wiki page created, I want below default content/excel is in the page (see screen shot). By this way, other user just need to input necessary information in below requested form to submit their content. how can I do this?
