How can I lock a SharePoint Power Apps form

I want to be able to lock a form once saved,

but still allow the person that created (created by) the item to still edit it

and only allow the supervisor [Supervisor Name] to edit the 4 form fields that they need to edit.

[Supervisor Name] is captured in the form.

How can I achieve this?
