How approver in approval workflow after rejection of document can attach another document

How approver in approval workflow after rejection of document can attach another document to main document can explain why he rejected and what changes should happen.

Now with out of the box approval workflow, Approver can just approved or reject document and is not possible another document/sketch/note can explain why this document for example has been rejected.
