Adding attachement to sharepoint list item using flow

I need to be able to add a word doc attachment to a sharepoint list item using flow.  Is anyone aware of any examples?  I am also looking for a flow to rename the word doc after it has been added as an attachment to a sharepoint list item.    Thanks.
