Since it’s just one missing file, check OneDrive on the web > Recycle bin first. For a personal account, deleted files normally stay there for 30 days. If the file was in Personal Vault, unlock the vault; you may also need to select Show Personal Vault items in the recycle bin. If the file still exists but its contents changed, use Version history instead.

    For a full OneDrive restore:

    1. Sign in at https://onedrive.live.com/ using the account that held the file.
    2. Open Personal Vault and complete verification to unlock it.
    3. Go to Settings > Options > Restore your OneDrive, then choose a date or custom restore point. This feature requires an eligible Microsoft 365 subscription and covers the previous 30 days.
    4. Review the activity list before starting. A full restore undoes changes across OneDrive, not just to the missing file, so download separate copies of newer files you want to keep.

If the vault error continues, the exact wording matters—it could concern unlocking the vault or a different account issue. Keep the vault unlocked and retry promptly in the same browser session. If it still fails, copy the error message, with personal details removed, rather than resetting or deleting the vault.
