Reflect app not working

I have been trying to use the new Reflect app in Teams and haven't been able to successfully send a Reflect card in a chat or channel. I've added the app, can open it in a channel or conversation, and make my question and setting selections. When I click confirm I get a brief loading icon and the window closes, but the card doesn't send. I've tried using the Desktop app on Windows and Mac, as well as the web app in Chrome and Edge and the issue remains the same. A colleague in my institution has successfully used Reflect, so I know its not an institutional setting. Any ideas what might be wrong?
