How to add the Forms App into a team meeting chat?

Hi!&nbsp;&nbsp;During a meeting I want to be able to start a survey. I know that is possible in a normal chat / Team channel. But I dont know how to add the forms app in

Does anyone experience Forms disappearing from the 3 dots as the number of attendees of a meeting increases?
