Forms not posting correctly to Teams
Having some issues when posting a form (poll) to teams. The setup of the Form looks fine, but as soon as you click send it posts multiple posts in the chat, & the poll is not interactive, instead shows the poll results - the screengrab of the chat feed is what everyone in the team sees once the poll is sent to the chat.
Is there something we are missing here (system admin) or is this a bug?

Just tried to recreate this, but is working and has the expected behaviour in terms of displaying the questions, displaying the polls results and only showing it once. See screenshot
It could have self corrected since you did it. However:
1.) Does it happen and is consistent in multiple teams?
2.) Does it happen and is consistent in multiple channels?
3.) Does it happen after logging in and out of Teams?
4.) Does it happen in the web client as well as the desktop client?
If yes to these, I would personally raise it to Microsoft support

Thank you for your help, apologies for the slow response was off for a few days. I tried a Form in Teams this morning and it is now working, not sure why it wasn't last week.
Answering your questions from what was occurring last week, however:
1.) Does it happen and is consistent in multiple teams? Yes
2.) Does it happen and is consistent in multiple channels? Yes
3.) Does it happen after logging in and out of Teams? Yes
4.) Does it happen in the web client as well as the desktop client? Yes
