Unable to view the option to add parameters to embed code in Microsoft Forms
Hi nithinvanam,
From your screenshot it looks like you are using the classic Forms rather than Forms Pro. The personalisation is only available with Forms Pro. If you use that, you will have the functionality. Then once you are using that, you can check out this: https://meganvwalker.com/add-personalisation-to-form-pro-survey/

Hi Megan_V_Walker ,
Thank you for your reply. I have a dynamics 365 CE license assigned to my id. As per the licensing, with a Dynamics 365 license, i should have access to Forms Pro. However when i sign-in from https://formspro.microsoft.com/en-au/, i am taken to Microsoft Forms and not Forms Pro. Is there a separate license that has to be assigned to the user to get access to Forms Pro or is there any other way to access Forms Pro? Thanks again for your help.

When you are logged in to Forms, can you click on your profile image and see if you have the option to switch to Forms Pro?

Megan_V_WalkerI can see from the view of my subscriptions that I have access to Forms Pro, but if I click on my profile I do not get the possibility to add it . I have also tried to buy it, but it telles me that I already have it- but how do I get access to Forms Pro ? Please hurry I need to send the survey today 😞

This should really be submitted as a ticket with Microsoft support to help you. Especially as you have an urgency. That would be the right channel for your high priority request.

I was able to switch to Forms Pro and was able to embed the form onto a local html file. However in the flow created to read the parameter, the "When a record is created" trigger for Form Pro Survey Responses doesn't seem to be triggered when my form is submitted. I used the list records action on the Form Pro Survey Responses entity to confirm that my response was created though.
This is my flow:
Is there something that i am doing incorrectly in the flow? As per your blog post the entity was Survey Responses but that doesn't seem to be available now. Please advice if there is any other way to retrieve the context data? Thanks again.

The difference in the name of the entity is because when I first started blogging about Forms Pro, Survey Response was the name of the entity. That was changed by Microsoft at a later time.
If your FLOW is not triggering, but there is a Survey Response in your CDS (D365CE) environment, I would open up a support ticket with Microsoft. I heard of this same issue from someone else yesterday. Perhaps it is a coincidence, but it's worth checking out. Of course, it is always worth checking it again today in case there were FLOW issues yesterday?
