Unable to use multi-select option for choice question

Hi,

I have a form that is used to collect details of new employees for onboarding. I have a choice question that I would like to be multiple choice however when I select the 'Multipl Answers' toggle it errors and I get redirected to the 'Something when wrong' page showing the sessions ID: 848979ff-12d5-4e40-bc5c-5f49eaa3f5c5.

I have some question in the form that were already set to multipl choice, I toggles one to off to see it would let me toggle it back on and it does not.

Has anyone else experienced this issue, am I doing something wrong or is this a bug?
