Trouble with Forms

Hi there,

I created a form under my profile a while ago, and have only just moved it to a Group Form so the rest of my team are able to access it.  However, they are now unable to open the Excel Spreadsheet and see the latest responses.  How can I convert it back to being a form under my user profile?
