This form can't be distributed as it is asking for personal or sensitive information.

We have a form that has been active and in use for nearly a year however today we have this error message. I can't find the message in admin centre that it is talking about it https://support.microsoft.com/en-gb/office/review-and-unblock-forms-or-users-detected-and-blocked-for-potential-phishing-879a90d7-6ef9-4145-933a-fb53a430bced?ui=en-us&rs=en-gb&ad=gb or anything and have even turned off the Phishing setting too. I've also removed any text that says Password or anything like that.

I'm not sure what else I can do apart from remake it and hope for the best?
