This form can't be distributed as it is asking for personal or sensitive information.

We have a form that has been active and in use for nearly a year however today we have this error message. I can't find the message in admin centre that it is talking about it here or anything and have even turned off the Phishing setting too. I've also removed any text that says Password or anything like that.

I'm not sure what else I can do apart from remake it and hope for the best?
