Teams and form results

Looking to see if there is a way to have a form or any app filled out and it would then auto load that into a teams chat/channel.  An example would be that if you need specific information for IT before they can assist. Wanted to see how i can add a template into any app that would auto open up a teams chat with the information they filled out.  I want to also track that data filled out behind the scenes to see who is submitting the requests.
