Started Getting "This form can't be submitted as it is asking for personal or sensitive information.

I just started getting this message on a new form I was developing. What does it mean and how do I get around it? What is the criteria being used to determine that a form is asking for personal or sensitive information? I am on Office 365 for Business and we would like to use FORMS but this could be a showstopper. Thanks for any ideas anyone has.

OK, I found the words login/password in my form and removed them and the warning went away. Is there a complete listing of the terms that are blocked so I can avoid having this happen unexpectedly?

What can I do if I am the one who is filling a form but I cannot submit because of this error?
