As you say, if you have already set this up, it shouldn't ask for the login details.
Are the login details Microsoft login or another thing?

Sounds like unusual behaviour (I'd confirm but for some reason Forms has error out on me).
I would suggest try logging a ticket with Microsoft about it (chat with your IT admin about it).
If anyone from Microsoft is reading this, they may also have a comment given it is a Preview version.
When it does come back online I'll have a play if this is an ongoing issue for you.
