Responses not being submitted to my form

I have a form here that seems to not be collecting any data. I had reports over the weekend that users were filling it in, but they have not appeared in the response section. I have tried to complete a submission myself and while it appears to work fine from a user's perspective, it does not show up in the response section. 

Can anyone advise?
