Record name becoming unchecked in Forms settings

I had an internal query from a Forms user who said that the 'Record name' field had become unchecked without them doing it.

While use error is always a possibility I was wondering whether anyone else had seen this behaviour.

It has not affected any other forms, and as that field is checked by default it seems like a system issue is unlikely.
