Public Facing Survey - Individual Response Links?

Question

Is there a way to make a public facing (no enterprise sign in required) form that allows you to provide an individualized survey link (to tie to demographic data)?

For example John Smith recently left the company and we'd like to send him a link to fill out an exit interview to his personal email. I'd like to be able to have the link identify that it is John Smith with out having John Smith enter his name/etc.

Is this possible?
