Privacy and sharing protected health information in Microsoft Forms.

Hello, 

I work for company that has access to patients protected health information. Would Microsoft Forms be a secure platform to use to gather this type of data? or is there the potential this information could be accessed by a third party? 

at the bottom of the form it states: Microsoft is not responsible for the privacy or security practices of its customers, including those of this form owner
