Permanent home for a form for ongoing input?

OldFatDog you could have a Microsoft Forms web part on your SharePoint homepage so they just go to that site, complete the form and click submit:

- honestly, if they are old enough to work, and hold down a job, I think they should be able to manage bookmarking the link to the form in their browser on their own right? If you still need to do something you can add links to sites so it would show in Office 365 when they log in.
