PDF Form into a Microsoft Form

Okay So I have a form that has been created in a PDF form and want it put into Microsoft forms the problem is it has to look very similar to the PDF version for professional reasons but want to use MS Forms as it is easier for people to fill out and for me to keep track of info. Is there a way to basically download the PDF version into MS forms? or at least the information page (that includes a header and information)

    shella1982 there is no way to download/convert the PDF into Forms. You have to create the form manually.

    Hi shella1982,

     

    It's possible the other way around (converting forms responses to pdf), but AFAIK you can't convert a PDF to Forms- it'll need to be manually created/formatted. There is a very good chance that the resulting Form may look better than the PDF.
